Apply for Services
Please submit the application form to apply for Healthcare Horizon's residential or housing support services. Once submitted, a member of our admissions team will contact you.
Eligibility Requirements:
- Must be at least 21 years old.
- Have a diagnosed intellectual or developmental disability.
- Receive funding for services from the DDA under the Community Pathways waiver, Community Supports waiver, or Supports-Only. Those who self-direct services or use private pay may apply for Housing Support Services only.
- Reside in Maryland while receiving services.
- Have healthcare and accessibility needs that can be responsibly managed by Healthcare Horizon Services Inc.

We offer a range of residential support services, including Supported Living, Personal Supports, and Community Living Group Homes. Our Housing Support Services operate separately but use the same application form.

Medicaid Funding
Most individuals receiving support from Healthcare Horizon have Medicaid funding through the Maryland Developmental Disabilities Administration (DDA). The eligibility and application process for DDA services typically takes up to three months. To apply for DDA funding, contact your regional DDA office.
